Liability Release

In order to properly cover their ministry and Equipt, all partners are required to have an “Agreement and Release of Liability” form signed by participants for off-site activities. 

This is mandatory for youth going to any kind of summer camp (parents sign for any minors under age 18) as well as ADULTS AND/OR YOUTH going to other offsite church or ministry sponsored/coordinated events (such as mission/work days, men’s/women’s retreats, etc.). You can check with us if you are unsure if an event requires this release.

If you let us know about any upcoming events you will need to use it for, we can make the submission tracking specific to your ministry (populating completed submissions into a subfolder of your shared Google Drive folder) to allow for your ease in record-keeping purposes.

We can also provide tailored PDF versions of the release for situations where you need to print paper copies for participants to sign.
